Sukanya Samriddhi Yojana – Sample Filled Application Form

I have already posted three articles for Sukanya Samriddhi Yojana and the queries regarding this scheme are not showing any signs of tiredness. People have been asking all kind of queries regarding various features of this scheme and sharing all kind of experiences here on these posts. The biggest problem they are facing is to find out which bank branches are opening this account and accepting deposits from the general public. Banks are showing their inability to open these accounts as they do not have any clue about the account opening process.

I also got a few comments in which people wanted me to post a duly filled application form so that they can also easily fill the application form for themselves. So, here you have the sample of a duly filled application form:

The application form is fairly simple and you can fill it in less than two minutes. Also, I have filled this application form to the best of my knowledge. So, if any of you find any discrepancy in it, please point it out to me and I’ll rectify it as soon as possible.

Please don’t forget to carry the necessary documents to open this account, which are as under:

* Birth Certificate of the Girl Child

* Identity Proof or Aadhaar Card of the Parent/Legal Guardian

* Residence Proof or Aadhaar Card of the Parent/Legal Guardian

* 2 Photographs of the Depositor/Parent/Legal Guardian

    Still bank do not have form to open the account. Kindly share the address of bank where I can get the form to open the above account for my daughter.

        Thanks for reply and surprising SBI bank do not know about this yojna. It is very pain full to followup the the bank people regarding this concern.

  4. I just want to know we can assign nominee while ofening Sukanya Samriddhi a/c in post office or Bank. Bank has no Details regarding this scheme . we have visiting number of bank.

    1. Nomination facility is not available with this scheme. Banks are still not opening these accounts. You’ll have to wait some more time for them to start opening these accounts.

  5. Spose one year money diposit in this yojna after that depositor expire then government continue this gonna or as it is stop.

    My daughter DOB is 23/12/2004, can I open an account under sukanya samriddhi scheme, please let me know.

    1. This scheme is for girls. Parents are supposed to deposit money to support girl child’s education and marriage. Girl child can withdraw maturity proceeds herself.

    At the time of opening the SSA, do we have to commit a fixed amount (say, Rs. 1,000 per month) to be deposited per month? Can I start with Rs. 1,000 per month, but for later months deposit Rs. 2,000 or any amount which cumulatively not exceed Rs. 1,50,000 per year?

    1. Hi Gitesh,
      It is not a monthly deposit scheme. You can deposit any amount between Rs. 1,000 and Rs. 1.5 lakh any time in a financial year. A fixed amount is not required to be deposited.

    I daily visit your blog and I think you are now a Jansewak doing Public awareness work. You will be blessed from all of those who got information through your blog. You answered each person individually . I am grateful to you for answering me ever positivelly. I see that ppl sometimes ask troubled questions but you never loose tour twmper in answering again and again on repeated questions.

    form me deposited ka name likhna hai phle child ka name pir father name likhna hair or form ka point 9 me data of birth of minor eska mtlb child ka dob likhna hair or data of majority me sir kya lukhna hai
